Can you post the form that you used to describe the model as well as the builder info. I have seen this form in Model Car Mag. many times. Because there is a Model Car Contest in San Bernardino, Ca. this weekend. If you are interested I can submit to Photos to you and the forms as well.

Or can you tell me what issues that this form is in so I can scan and make copies.
